Announcement: Changes to our Triage System coming 12th January 2026

We have been listening to your feedback; while some patients are enjoying the ease of access provided by our triage form, others are struggling with the length and usability of the form.

As a result, on 12th January 2026 we will be moving to a new form - Accurx Patient Triage.

 

What is Changing?

The only thing changing is the form that you use to submit your request.

The old form had lots of questions and "pathways". This information helped the system to book you an appointment with an appropriate clinician in the right timeframe.

However, over time we found hat this didn't work as we expected, so all requests went into an inbox to be reviewed by a clinician. 

With all forms going into an inbox, there was no need for the form to be so complicated!

As a result, we have chosen Accurx Patient Triage, which has a much simpler form; only 4 manatory questions, and 3 optional questions to help us triage your request.

Form with 7 questions; Describe the problem, attach a photo (optional), How long has it been going on for? Have you tried anything to help? Is there anything you're particularly worried about (optional), How would you like us to help? What are the best times to contact you (optional).

 

 

How will I request an appointment?

From 12th January 2026, we will be using Accurx Patient Triage. 

  • Click the booking link on our website (this is linked in the banner on our home page)
  • Select the appropriate request type (either a medical request or admin request)
  • Answer the questions prompted by the form.

At the end of the form, input your personal details so that we can find you in our system. Once submitted, your request will be reviewed by our triage team who will contact you about your request. 

If you aren't able to submit your form digitally, you can still call the practice on 01342 327555 and speak to our Patient Services Team, who will complete the form on your behalf. 

Alongside the online and telephone options, you can still book an appointment by visiting the practice. In-practice booking stations are available in Reception for patients to submit their requests.
