NHS Talking Therapies - Long term health conditions

 

Talking Therapies is a free and confidential NHS Service avalible to adults registered to a GP in West Sussex aged 18+ who would like some support with one or more of the diagnosed conditions listed below.

  • A cardiac (heart) condition
  • Uncontrolled or severe asthma
  • Musculoskeletal (MSK) conditions
  • Irritable bowel syndrome (IBS)
  • Diabetes - type 1, type 2 or gestational
  • Chronic obstructive pulmonary (lung) disease (COPD)
  • Long covid - ongoing symptoms 12+ weeks

 

Living Well Programme 

The online ‘Living Well’ course is designed for people living with long term health conditions, such as Diabetes, Heart Disease, Arthritis, Depression, Asthma etc. The goal is to help you to learn new skills and techniques to better manage your condition.
Among other things, the course explores:
  • Managing Common Symptoms such as fatigue, pain, isolation, physical limitations etc.
  • Goal setting & Problem Solving
  • Healthy Eating
  • Physical Activity
  • Apps & Online Resources
  • Accessing NHS services
  • Relaxation Techniques
  • Positive Thinking
